چکیده

According to the World Health Organization, each year 10 million people are diagnosed with tuberculosis for first time and 1.5 die from it. The death rate this disease has increased in world more than ten years. Unfortunately, Ukraine is TOP-10 countries largest number of cases among population. Only December 2021, 1,229 were registered Ukraine. To date, course process undergone significant changes. infiltrative form (IF) pulmonary accounts majority new cases. Standardized treatment (60 doses intensive phase 120 continuation phase) not always sufficient effective recovery requires prolongation. That why it necessary study predictors that maximally reflect need therapy objective: analyze dynamics clinical, laboratory radiological parameters patients IF newly (NDPT) under conditions varying effectiveness. Materials methods. men working age NDPT examined KNP Kharkiv Regional Council “Regional TB Dispancer N1” during 2019–2021. Patients divided into two groups: Group 1 (n=89) included positive clinical process, as a result clearing Mycobacterium (MBT) sputum; 2 (n=31) weak dynamics, which was extended 90 doses. Comparison data at beginning end different efficiency performed. conducted accordance requirements good practice, Declaration Helsinki Medical Association, approved by local ethic committee Academy Postgraduate Education. Results. An analysis showed decrease immuno-inflammatory indicators levels (C-reactive protein, IL-4, IL-10, circulating immune complexes; CD4/CD8 ratio) pronounced group who did At same also characterized increase level IFN-γ treatment, could indicate activation cellular immunity together IL-4 IL-10 indicated suppression humoral immunity. Due predominance over humoral, macrophage their phagocytic activity accelerated, MBT elimination much faster efficient patients. Changes cytokine observed 1, after but 2, demonstrated signs dysregulation due continuing specific inflammatory process. Conclusions. Tuberculosis remains one global health problems. general trend spread mortality throughout urgent efforts detection disease. In standard less case slow insufficient CRP, γ-INF, ratio associated cavities healing, spreading These needed prolonged regimen.
